You are entering into a three month/six session coaching program, from today through the end of January 2020. This will consist of standing biweekly coaching sessions (about 60 minutes), as well as additional spot coaching or Power Hour support sessions as requested.
The agreed-upon rate for this is $150 per month, paid monthly.
On or around February 5th, 2020, we will have a new co-creation call where we will revisit how things have gone, what you are creating and what the appropriate level of support will look like, and we can either complete coaching, continue as we have been going, or create something new from there as mutually agreed upon.
Components & Intentions of this coaching container will include:
A dynamic somewhere between a hug and Nun-With-Ruler, but to a higher gradient than before.
A focus on business development, especially creating more income & creating more time and joy and followthrough
Perhaps something else I can’t remember right this moment because I forgot to bring the correct notebook along to the library (sorry)
Client Agreement
The following is an agreement between You, the coaching client, (“Client”/“You”), and the coach, Becky Augustine (“Coach”).
A Private Coaching Program
You are entering into a private coaching program whereby Becky Augustine will provide high level coaching to You over a period described above.
By entering into this contract, you retain the locked-in private coaching rate from the time of your first coaching arrangement.
Previously Locked-In Rates
BIWEEKLY: $2750, or six payments of $550, and includes 2 standing sessions per month and occasional spot coaching as requested.
WEEKLY: $4825, or six payments of $965, and includes 4 standing sessions per month and occasional spot coaching as requested.
If you are paying in monthly installments you hereby agree to remit the monthly payments via preauthorized automatic electronic debit to your banking account or credit card, which will be charged every month, and continuing until the end of the coaching term, or if month-to-month until you terminate the coaching agreement with 30 days written notice. Payments are not refundable for any reason. Payments are due upon scheduling regardless of whether or not you fully and faithfully participate.
Cost of telephone calls: Unless otherwise agreed upon, You are responsible for initiating the call to any session, and You bear the cost of the telephone call. An audio Internet link is also available for clients who prefer this.
What To Bring: You are not required to bring anything to coaching sessions. I recommend you bring paper and pen, along with your previous notes and notes about your week, and anything you need that helps you to be comfortable and focused.
Work Between Sessions: There are often assigned tasks for you to complete between sessions. I refer to these assignments as “Weekly/Monthly Practice Areas.” I recommend that you complete them. You cannot expect the same results from your participation when these tasks are not completed.
Confidentiality
Your coaching is kept in the strictest professional confidentially. Of course, law does not protect the confidential communication between coach and client, as a coach is neither a health care professional nor a legal professional. From time to time, I may request your permission to record a session or to allow another coach to participate in the session for training purposes. These recordings or participation are not used for any purpose other than training and are kept confidential.
I will only suspend this confidentiality agreement in the following areas:
-You report imminent suicidal or homicidal ideation
-You report abuse or neglect of a child, dependent, or older adult
-I am ordered by a court of law to do so
-You report anything that causes concern for the health and safety of you or others
The Coach promises the Client that all information provided to the coach will be kept strictly confidential. Any client stories which are shared on blog, video, tele course or other medium for marketing or coaching purposes, or within a program or course for sale, will be pursuant to client approval and/or will be made anonymous and fictionalized to protect the identity of the client. Coach has the right to use case studies or examples of Client’s situations and results or Client testimonials in future work but will never make reference to Client’s full identity, unless provided with written approval from the Client.
The Coach reserves the right to record and/or photograph any session for review by a confidential certification committee that has the same ethics and confidentiality agreements, for ongoing coach training and certification purposes only. The Coach reserves the right to use any photograph/video, without the expressed written permission of Client, however to ensure the privacy of Client, images will not be identified using full names or personal identifying information without written approval from Client, and no Session will ever be made public. If Client does not wish to have their session recorded for training purposes must make their wishes known to Company in writing.
In summary, I will uphold the highest standards of ethics and confidentiality in our coaching relationship.
Coaching Relationship
Throughout the working relationship, the Coach will engage in direct and personal conversations. The Client can count on the Coach to be honest and straightforward in asking questions and making requests. The Client understands that the power of the coaching relationship can be granted only by the Client, and the Client agrees to do just that: have the coaching relationship be powerful. If the Client believes the coaching is not working as desired, the Client will communicate that belief and take action to return the power to the coaching relationship.
Health And Well-being
Please note that you are responsible for your health and well-being at all times during coaching. If you have any questions with regard to your ability to participate in coaching, please consult your doctor or health professional prior to the start of your coaching.
Coaching is designed to provide access to power and freedom in your life. Coaches do this through distinguishing the things that either stop you or are inconsistent with the results you are committed to producing. Coaching is not therapeutic in nature or design. Coaching is neither health care nor rehabilitation, nor is coaching anything medical in nature. Neither is coaching designed to be curative, healthful, or even remotely linked to your physical and mental well-being. It is simply a guided conversation with recommended actions. If you have health or well-being concerns, I recommend that you consult a health professional. If you have any questions about this, please consult with your health professional prior to the start of your coaching.
Informed Consent
I understand that I am responsible for my own participation in coaching, and for my own well being. I have not taken on coaching as a substitute for the services of a health professional. I have not taken on coaching to handle a physical, emotional, mental or addictive problem. I am clear that no part of coaching is presented by or under the supervision of health professionals.
I assume for myself and my heirs, family members, executors, administrators and assigns, all risk of physical injury and emotional upset, which may occur during or after coaching. I agree to hold Becky Augustine, Accomplishment Coaching, its agents & officers harmless from any and all liability arising out of my participation in coaching.
Payment
Coaching will begin after payment has been received. Contact coach for current payment options. Thirty days written notice is required to initiate cancellation of month-to-month contracts so as to do complete work around your projects and set you up for future wins.
Refunds
Coaching is, by design, a confronting process. At times, a client may feel that s/he is unable or unwilling to move forward with a project as fear or discomfort arises. While coaches understand that this may be a normal part of the process, I am committed that you deliver results on your projects and not be stopped by your fears or concerns. I cannot and do not offer refunds of any monies paid. Simply, the coaching relationship is a professional engagement. Once your fees are paid, the services are available. Of course, you are free to cancel the engagement at any time, with thirty days written notice. This notice gives your coach time to complete the progress to date and ensure that you are set up powerfully to move into what’s next for you.
In the case that the coach should become suddenly unavailable (ie: medical emergencies), the client has the option of working with an Accomplishment Coaching-trained partner coach, or postponing sessions. In the unlikely event that the coach should become suddenly unavailable for more than two months the client has the right to a refund for the duration of cancelled services if they cannot be rescheduled.
Cancellation Of A Session
Circumstances may require a session be re-scheduled by either the coach or the client. As availability is limited, advance notice of cancellations is required to reschedule. A session that is not cancelled with a minimum of 24 hours may not be rescheduled, based on availability. Coach reserves the right to reschedule any coaching calls to accommodate holidays, health, travel and other needs as necessary.
Any session where notification is provided can be re-scheduled, as time permits. From time to time, this re-scheduling may result in more than one session per week, especially in the event of vacations and holidays. These re-schedules are more numerous during busy holiday seasons, so your flexibility and advance notification are appreciated.
Becky Augustine and Becky Augustine Coaching are under no obligation to refund or reschedule a coaching session if You fail to complete or appear for any or all of your private or group coaching sessions. In addition, all private coaching sessions included in the program must be used before the completion of the term of the Coaching Agreement, or be forfeited, unless otherwise noted in writing.
Copyright Protection
The coaching concepts, materials and information are property of Becky Augustine Coaching and/or Accomplishment Coaching and contain copyrighted materials. You are asked to agree that you will not use or reuse our materials for any financial gain or otherwise distribute or remarket to any third parties. You are forbidden to reproduce the materials without the express written permission of Becky Augustine and Becky Augustine Coaching.
Materials Agreement
I understand and agree that all of the materials and information provided and presented to me in my coaching sessions are the property of Becky Augustine Coaching and/or Accomplishment Coaching (the “Materials”), and in many cases are protected by copyright, trade secret or other proprietary laws. I also understand and agree that this agreement provides for the protection for the Materials presented that extends beyond federal, state, local or foreign copyright laws or treaties and that by signing this agreement I am agreeing to be bound by the terms and conditions herein, even if such terms and conditions extend beyond such laws.
Materials Ownership And Protection
Notwithstanding any provision to the contrary contained in this Agreement, it is understood and agreed that the Materials and all intellectual property rights associated therewith are, and will remain, the sole and exclusive property of Accomplishment Coaching and its suppliers and licensors and that no license, right, title, interest in and/or to such Materials is granted to client by virtue of the coaching services provided. Accomplishment Coaching will retain all right, title and interest to any modifications made to the Materials, derivative works derived from the Materials, and/or incorporating the Materials, if any. Client hereby makes all assignments necessary to accomplish the foregoing.
Client agrees that it may not copy, modify, adapt, translate, rent, lease, sublicense, loan, resell for profit, distribute, time-share, or create any derivative works of the Materials nor allow or sell access to the Materials, or use the Materials for the benefit of, any distributor, reseller, sub licensor, aggregator, or remarketer of any kind, including any other coaches or course providers. Client may not offer to a third party any sample or test on the Materials, including media, and will reproduce the legends on any reproductions.
Limitation of Liability
While Becky Augustine is absolutely convinced that you will derive great benefits from our coaching relationship and that You should become as connected with your Highest and Greatest Self as You envision, We of course cannot represent or guarantee that you will attain certain level of relationship, personal success or emotional wellness as a result of our Coaching Program. As with any small business or career, each individual’s success depends on many factors, including your personal motivation, time commitment, how effectively you implement the strategies taught, and your efficiency in following up on each phase. The cost of this Program is an investment in yourself, and, as with any investment, there is both great potential for a great return on your investment, and a risk there is no return on this investment. We can guarantee, however that you WILL learn a lot of usable and translatable strategies for success that, when followed by others, have achieved Life Changing results.
Disclaimer
I understand that I am working with Becky Augustine for professional Ontological Life Coaching at the agreed fee. Professional coaching is distinctly different from counseling, psychotherapy or psychoanalysis and does not deal with the diagnosis or treatment of emotional problems. Since professional coaching does not constitute medical consultation or treatment, health insurance does not apply. These fees may be considered deductible business expenses.
Declaration
As Becky Augustine, the Coach, will be making suggestions, recommending practices, advising, taking the Client through exercises and meditations, and teaching the Client the communication and personal techniques developed by the Coach and by Accomplishment Coaching. Unlike the co-active coaching aspects of working together, where the Coach may ask questions and walk alongside the Client on his/her journey, rather than tell him/her what the Coach believes is their best course of action, teaching and consulting does require the Coach to lead and guide the Client. It requires the Coach giving information and instruction to the Client. As an ordained (non-denominational) minister, the teaching and consulting from Becky Augustine, therefore, is to be considered pastoral advice. The Coach is not a therapist, nor is Coach licensed by the State of Maryland in any way. The decision to accept or decline information, instructions, guidance, suggestions, and teachings from the Coach is the Client's to make.
The Coaching Program is not intended to provide legal, financial or health-related advice. As the Client, I understand that Becky Augustine, Becky Augustine Coaching, and the Coaching fall under the category of Life Coaching, and I further I understand that life coaching is a comprehensive process that may involve all areas of my life, including work, finances, health, relationships, education, sports and recreation. I acknowledge that deciding how to handle these issues and implement my choices is exclusively my responsibility.
As the Client, I understand and agree that I am fully responsible for my well-being during my coaching sessions, including my choices and decisions. I am aware that I can choose to disregard any advice given during public or private sessions, whether from and intuitive, coaching, health, or business standpoint. I recognize that coaching, mentoring, and intuitive guidance is not psychotherapy and that professional referrals will be given if needed.
As the Client, I understand that Becky Augustine and the Coaching Program does not treat mental disorders as defined in the DSM-V by the American Psychiatric Association. I understand that the advice and information given in the Program, whether written or spoken, is not a substitute for counseling, psychotherapy; psychoanalysis, mental health care or substance abuse treatment and I will not use it in place of any form of therapy. I agree that I am well adjusted, mentally health and ready to receive coaching and intuitive services. As the Client, I agree that if I am currently in therapy or otherwise under the care of a mental health professional, I have consulted with this person regarding the advisability of working with a life coach or spiritual teacher, or receiving intuitive advice, and that this person is aware of my decision to proceed with the life or spiritual coaching relationship.
By purchasing and entering into any coaching work with Becky Augustine, the Client hereby agrees to this disclaimer.
